At Elite Primary Care, we make house call visits to people who have a hard time getting to their doctors appointments. We specialize in providing comprehensive acute and chronic disease management using the latest technology, including referrals and coordination of care in the comforts of the patient’s home.
The best house call medical care is delivered by a team. Our home-based care team includes knowledgeable physicians, nurse practitioners, physician assistants, and medical assistants. We provide advocacy, access, continuity, and compassion high quality care to enhance the quality of life to our patients.
What We Do
We deliver customized medical care to those who have trouble leaving their home. The patient will be cared for by one of experienced staff who will visit you in the comforts of your home to hear about your medical history, and any concerns you may have. A provider will visit you monthly or more often if you need medical attention. We can also arrange for another type of support, such as nursing, pharmacy, or other basic needs. We can help coordinate the care between your specialists, such as cardiologist or pulmonologist.

- Referrals to All Home Care Services and Medical Equipment including Physical Therapy and Rehabilitation Services
- Physician’s Review for Authorization Including Home Health Authorization and Care Plan Oversight
- Emergency Room and Hospital stay follow up
